Exercise cannot straighten a scoliotic spine, but the right movements can meaningfully reduce pain, improve muscle balance, and support better posture. These six exercises are appropriate for many individuals living with scoliosis, though any new exercise program should be discussed with your spine specialist before you begin.

Living with scoliosis does not mean giving up on physical activity. In fact, staying active is one of the most important things a person with scoliosis can do for their long-term spinal health. Targeted exercises strengthen the muscles that support the spine, ease the discomfort that often accompanies a lateral curve, and help patients maintain the mobility and posture that reduce strain on the vertebrae over time.

The exercises below are commonly recommended for individuals with scoliosis. They are low-impact, can be performed at home, and focus on the core stability, flexibility, and muscular balance that the scoliotic spine depends on most. That said, scoliosis is a highly individual condition, and what is appropriate for one patient may not be right for another. Always consult with your physician or physical therapist before adding new exercises to your routine.

1. Pelvic Tilts

Why it helps:

The pelvis is the foundation of the spine. In individuals with scoliosis, pelvic imbalance often contributes to uneven loading along the lumbar spine. Pelvic tilts strengthen the lower abdominals and lumbar muscles while gently mobilizing the base of the spine.

How to perform:

  • Lie on your back with your knees bent and feet flat on the floor.
  • Gently flatten your lower back against the floor by tightening your abdominal muscles and tilting your pelvis upward.

4. Latissimus Dorsi Stretch

  • Stand with your feet shoulder-width apart.
  • Raise one arm overhead and gently lean your torso to the opposite side until you feel a stretch along the side of your back and into your armpit.
  • Hold for 20 to 30 seconds, then switch sides.
  • Repeat two to three times per side.

Pay attention to which side feels tighter. Spending additional time stretching the tighter side can help address the muscular imbalance associated with your curve.

5. Seated Trunk Rotation Stretch

Scoliosis involves not just a lateral curve but also rotation of the vertebrae. This seated stretch encourages gentle rotation of the thoracic spine, improving mobility in the area most commonly affected by the curvature and helping relieve the stiffness and discomfort that rotation restriction can cause.

How to perform:

  • Sit upright in a firm chair with your feet flat on the floor.
  • Cross your arms over your chest or place your hands on your shoulders.
  • Slowly rotate your upper body to one side as far as is comfortable, keeping your hips facing forward.
  • Hold for 10 to 15 seconds, then rotate to the other side.
  • Repeat five times per side.

Sit tall throughout the movement. Allowing the spine to slump reduces the effectiveness of the stretch and places unnecessary load on the lumbar discs.

6. Glute Bridges

Why it helps:

Weak glutes place excess demand on the lumbar spine and contribute to pelvic instability. For scoliosis patients, strengthening the glutes reduces compensatory strain along the lower back and helps the pelvis maintain a more level, symmetrical position as a foundation for the spine above.

How to perform:

  • Lie on your back with your knees bent and feet flat on the floor, hip-width apart.
  • Press through your heels and lift your hips toward the ceiling, with your shoulders flat on the floor, until your body forms a straight diagonal line from your shoulders to your knees.
  • Squeeze your glutes at the top and hold for two to three seconds.
  • Slowly lower back down.
  • Repeat 10 to 15 times for two to three sets.

If one side of the pelvis tends to drop during the movement, focus on engaging both sides of the glutes evenly. Your physical therapist can help you identify and correct this pattern if it is occurring.

When to Stop and Seek Guidance

Even gentle, low-impact exercises can become problematic if they are performed incorrectly or if they are not appropriate for your specific curve. Stop exercising and contact your spine specialist if you experience any of the following:

  • Sharp or worsening pain in the back or limbs
  • New or spreading numbness or tingling
  • Sudden weakness in the legs or arms

Can these exercises replace surgery?
